I hereby give my consent to the processing of personal data by the company HIPOKRAT sro, with its registered office at Bernol\u00e1kova 369\/1, 908 51 Hol\u00ed\u010d, I\u010cO: 36358703 for the purpose of creating and maintaining a user account, through which the user as the affected person is training events in which he has participated and \/ or the provision of other operator-specific and user-defined user account functions.<\/p>

The processing of personal data will be carried out for the duration of the user account, but no longer than for a period of 10 years from the date of the last login to the user account. Consent to the processing of personal data can be revoked at any time in writing, which terminates the user account.<\/p>
